Changes in name, address, and telephone number must be submitted in writing to the SGMP Membership Office. Print or download the Name Change Form below and complete. Please print or type! Be sure to sign your form and provide payment information! It will NOT be processed unless it is signed and payment information provided. Note: Effective May 1, 2008 there will be a $25.00 fee assessed for submitting a name change for all members. Name Change Form
